Type Examination Certificate

Example of Test Type Examinations Certificates issued by different Notified Bodies.

The following to be noted on such a certificate:

    • Notified Body name, number and logo
    • Certificate number
    • Name of Manufacturer
    • Product Type
    • Product Range/Series
    • Product Sizes
    • Testing Standards
    • Directive/Regulation used
    • Signed and dated by Notified body
    • Validation of Certificate ±10 years

Declaration of Conformity

Declaration of Conformity issued by the Manufacturer stating minimum requirements:

    • Manufacturers name and logo
    • Type Examination Certificate Number used for the type of equipment applied for
    • Notified Body
    • Standards tested against as per Type Examination Certificate
    • Signed and date

Application form

Section 1 – Indicate if new application, renewal or modification to existing permit application.  For renewal please use – and not a / on numbering

Section 2Applicant Details (the applicant is the sole legal entity to be recorded as the Permit Holder with a domicile in SA)

Section 3Responsible Person – person appointed to administer the application on behalf of the business

Section  4 – Quality Management System Information – indicate if the Manufacturer of the equipment has an ISO 9001, make use of Conformity Assessment or None of the above

Section 5 – New Application / Renewal Of Permit – populate all fields as per the certification provided.  Should the Manufacturer not be on list, select “Other” and type name in open field

Section Certification – upload all Test Type, Type Approvals, Declaration and rest as per tabs.  All Punctuations marks, special characters, symbols and diacritical marks and accented letters will prevent you from submitting the application with reasons that document “still uploading” or “error format” message 

Section 7 – Declaration by Applicant – Terms and Conditions to be signed and dated
